Most health coaching lives in a silo. You help someone set goals, maybe they follow through, maybe they don't — and you never really know if it mattered clinically.
This is different.
At Mederva Health, you're embedded inside medical practices, working directly with physicians to help patients reduce their risk factors for chronic disease. Your coaching isn't a nice-to-have. It's part of the clinical plan. When a patient's A1C drops or their blood pressure normalizes — that's your work showing up in the numbers.
The role
You'll deliver brief, high-impact coaching sessions (15–30 min) to patients flagged by their doctors for modifiable risk factors. Think: the patient whose labs are trending in the wrong direction, who knows what they should do but isn't doing it. That's where you come in.
- Coach patients virtually via video, phone, and secure messaging
- Help patients identify the small behavior changes with the biggest clinical impact
- Set SMART goals and build tiny habits across diet, exercise, sleep, stress, and emotional health
- Use motivational interviewing and behavioral activation to drive real follow-through
- Work alongside primary care physicians who rely on your input to guide patient care
- Track patient progress and adapt your approach based on what's working
- Our AI platform handles documentation and admin workflows so you stay focused on the patient.
Requirements

NBC-HWC certification is a plus. Other credentials welcome but not required.
