The Behavioral Health Intern - Field Based Health Coordinator will be the primary care manager for a panel of members with complex medical/behavioral needs. Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.
The Behavioral Health Summer Internship Program is full-time, 40 hours per week for 10 weeks starting June 2, 2026.
This position does travel to visit community centers, facilities, community activities, and patient homes across a designated territory. Mileage reimbursement will be provided. This role is hybrid as the intern will work from home when not in the field.
The Behavioral Health Summer Internship Program is designed to provide students with hands-on experience in areas within the behavioral health field beyond traditional clinical settings. During this program, you will gain exposure to patient care needs through the healthcare continuum, enrichment opportunities such as the A Day in the Life series, learn about Enterprise Foundations, and build connections through networking and a student cohort community.
Primary Responsibilities:
As a Behavioral Health Intern for Community & State - Hawaii Health Plan, you will learn how to manage the care of members in their homes or other community settings by completing in-person health assessments and coordinating patient care, focusing on disease management, and keeping members healthy and independent. Under the supervision of our RNs and providers, you will learn how to:
- Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needs
- Develop and implement person centered care plans to address needs including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management and member safety in alignment with evidence-based guidelines
- Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan
- Provide education and coaching to support member self-management of care needs and lifestyle changes to promote health
- Support proactive discharge planning and manage/coordinate Care Transition following ER visit, inpatient or Skilled Nursing Facility (SNF) admission
- Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team
- Attend community fairs and outreach events held during the week or weekend

Required Qualifications:
- Currently enrolled in, or recently graduated from, an accredited bachelor's degree program, with graduation expected within 12 months of the internship start date (before or after)
- Currently pursuing, or have completed, a degree in Psychology, Social Work, Counseling, or Nursing
- Current Basic Life Support (BLS) certification
- Experience with MS Office, including Word, Excel, and Outlook
- Available to work full-time, 40 hours per week during the duration of the summer internship program (from June 2 - August 7)
- Legally authorized to work in the US without any restrictions. Proof will be required
- Must be eligible to work in the U.S. without company sponsorship, now or in the future, for employment-based work authorization (F-1 students with practical training and candidates requiring H-1Bs, TNs, etc. will not be considered)
- Access to reliable transportation that will enable you to travel to community centers, facilities, community activities, and patient homes across a designated territory
Preferred Qualifications:
- 3.0 or higher cumulative GPA
- 1+ years of Clinical Experience
- Knowledge of clinical terminology
- Demonstrated oral and written communication skills
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $21.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
